Ganz ganz Grausames Spiel, NICHT KAUFEN!
Not Recommended — for now I work in emergency medicine, so this was an easy sell for me. Launch didn't go smoothly. A day-one patch adding language support broke the game — pressing play from the main menu just looped the opening credits. It was sorted about twelve hours later, though it came and went without any developer comment. Once running, my first hour served up the same patient three times and the same injury and past medical history seven times, with one new injury in between. There may be more variety further in, but a sim needs to show its range early and this doesn't. The design issue is the bigger one. You don't really diagnose anything. The tablet gives you a shortlist of possible conditions, tells you which tests to run, then tells you what to do with the results. It's a guided sequence rather than a puzzle. The interesting part of emergency medicine is working a differential — deciding what's worth ruling out, committing to a call, occasionally being wrong — and none of that is asked of you here. It looks the part more than it plays it, which puts it closer to arcade than sim. There's a good game in this. The setting works, the presentation is solid, the framework is sound. It needs real depth on the diagnostic side and a much wider case pool to hold anyone past the first few shifts. At £24.99 I'd wait and see what the first run of updates brings. What I'd like to see: Observations. There's no way to check a patients vitals other than the paramedic saying "blood pressure is slightly high". In a medical sim, vital signs aren't a feature — they're the foundation everything else sits on, and they should have been in on day one. Freedom in diagnosis and testing. Let the player choose their own tests and follow their own reasoning, with consequences for getting it wrong. Ordering unnecessary tests should cost you time or resources, not be quietly corrected. Cannulation and bloods on any patient. At the moment every patient ambulance or waiting room already has one. Being able to gain access and take bloods on anyone would open up a huge amount of the actual work. Also adding colour topped blood bottles designed for certain tests would be a nice feature. A proper ambulance entrance. The same paramedic stands outside the front doors every time with the patient already on a hospital bed. An ambulance bay, a handover, some variation in crew — it would do a lot for how alive the department feels. Happy to revisit if it develops.

Overall, the game is okay, but it quickly starts to feel lacking in depth. There is basically no real story — you simply follow the instructions on the tablet step by step, with very little influence over how things unfold. After a while, the gameplay becomes extremely monotonous and repetitive. You keep performing the same examinations and the same procedures over and over again, so the different cases barely feel different from each other. There is also no time pressure and almost no real decision-making. There is no meaningful prioritization of patients or examinations. Whether I do the ECG first, the ultrasound first, or something else makes absolutely no difference. There are no consequences for choosing the wrong order and no feeling that you actually need to assess the situation quickly and decide what is most important. The game has potential, but right now it feels more like following a checklist than a proper simulation where you have to analyze situations and make meaningful decisions.
The game has strong potential, but right now it sits in an awkward middle ground: it doesn't feel like a deep simulation, yet it lacks engaging gamification. With further development, it could easily balance both. Some suggestions: ECG Placement: Replace the current "on rails" placement with a mechanic where the player places electrodes based on placement markers (bonus points if you add tangled lead annoyances!). On removal, don't make players peel them off one by one, modern ERs use vacuum electrodes that detach automatically when done. Medication Prep: Walking between two separate closets for a vial and a syringe feels tedious rather than realistic. Urgent meds should come in pre-filled syringe ampoules, while non-urgent meds could feature a sterile draw-up minigame. Time & Urgency: Introduce time limits and progressive deterioration if a patient waits too long or receives incorrect care to add some adrenaline. Handovers: Upgrade handovers or consults using the standard ISBAR / ISOBAR format instead of the current setup. (both receiving one at admission and giving one handing the patient over to another department) Patient Assessment: The game currently heavily favors starting with a detailed history. It would be far more realistic and engaging to start with a broad opening question and then pivot directly into the ABCDEF approach. TL;DR: High potential. Needs refinement on either the simulation rigor or the gamification loops to truly shine.
Abwechslung sucht man vergebens. Polytraumen sucht man vergebens.
The game seems very broken still. Can not pull up the inventory, can not grab painkillers from the cabinet which has hard-locked me from progressing. I will have to wait for an update for them to fix this... and if one does not come in the next 2 weeks, I will be refunding my purchase.
