Prices
The United States storefront in every case, so this compares stores and not countries.
Prices
Each in the currency that store quoted — not converted: the gap between two rows is the publisher's pricing, not an exchange rate.
Every market we collect.
Charts
Each storefront's own chart, in its own country — the closest public signal to sales where none is published. A country missing from a list is one we don't collect there, not one where the game doesn't chart.
No United States chart position — it charts only in the markets below.
Discounts
Every price cut of more than 20% we have observed, in each market that quoted one, with this game's chart position on either side of it. These are episodes, not a model: a sale is one event, and one event cannot tell you what the next one will do.
Reviews, worldwide (the counter carries no country): 1.14/day in the week to Aug 23 → 5/day in the week after.
For scale, across Steam · Top Sellers · Brazil over the last 30 days: the 26 chart moves that followed a price cut of more than 20% went a median of 7 places towards #1, and the 2,402 moves with no price cut behind them went 2 away. One store, one chart, one country — it is the only place we have enough observations to say even that.
Prices outside the United States have been collected since 6 August 2026 and chart positions since 19 July 2026, so a cut older than that has no market to compare and a market that joined later has no history yet. Nothing here is converted between currencies.
🏆 The Medical Analyst's Verdict: Realism over Rib-Cracking Comedy 🏆
TL;DR Summary This is the ultimate hospital management sim for the player who thinks Theme Hospital wasn't technical enough. Project Hospital demands you master three distinct disciplines: architecture, logistics, and medicine. Your core challenge is building a highly efficient, multi-story facility, contracting the right insurance companies, and then manually diagnosing complex patient cases by ordering the correct sequence of lab tests and procedures. It has a notoriously steep learning curve, and the user interface and staff AI can feel finicky and counter-intuitive at first. However, the sheer depth of the diagnostic puzzle and the satisfaction of watching your optimized departments run smoothly make the initial struggle entirely worthwhile. If you love deep simulation and intricate systems, and are willing to put in the study time, this is an easy Recommended.

🩺 Core Analysis: The Three Pillars of Hospital Management

Pillar 1: The Architectural Challenge
The building phase is not just decorative; it is a critical game mechanic. A core challenge is designing efficient layouts that minimize staff travel time. For example, failing to place the Blood Collection Room near the Diagnostic Lab or putting your X-Ray machine on the wrong floor will result in patient collapse due to delays. The design tools are powerful, allowing for multi-story, highly specialized wards, but be warned: object rotation and placement can be frustratingly precise, which is more of a UI quirk than a flaw.
Pillar 2: The Diagnostic Puzzle
This is where the game shines and truly earns its "Medical Sim" tag. In the optional, but highly recommended, "Doctor Mode," you don't just click "Auto-Diagnose." You receive a patient with symptoms like "fatigue" and "abdominal pain," and must strategically select and sequence tests—from a simple blood panel to a complex MRI—to narrow down the actual disease. Guessing wrong wastes precious time and money. Correctly diagnosing a rare case through logical, methodical testing provides an immensely rewarding, detective-like experience.
Pillar 3: The Logistics and Staff AI
To thrive, you must become a micro-manager. The staff AI, while functional, will sometimes make questionable prioritization decisions, such as a doctor examining a low-risk flu patient before addressing a trauma case with heavy bleeding. This is a deliberate design choice that forces the player to actively monitor and intervene (e.g., overriding doctor assignments or adjusting staff shifts). The deep scheduling and skill-training systems allow you to fine-tune your staff into true specialists, but they require constant vigilance.

✅ Pros & ❌ Cons
✅ Pros

Highly realistic and detailed medical simulation that surpasses its genre competitors.
Deep and flexible architectural design tools for building complex hospitals.
The diagnostic puzzle system in Doctor Mode is genuinely challenging and satisfying.
Robust system for staff specialization, scheduling, and skill development.
Strong replayability through different scenarios, insurance contracts, and challenges.
❌ Cons

Extremely steep learning curve; the tutorials only cover the bare minimum.
Staff AI can be illogical and requires constant player micromanagement (not always a "set-it-and-forget-it" sim).
User Interface (UI) and object placement controls can be finicky and slow to navigate.
Lack of freedom for customizing keybinds is a minor but persistent annoyance.
The base game can feel a little sparse without some of the excellent, complexity-enhancing DLCs.

⚙️ Technical Assessment
For a complex simulation title, Project Hospital is remarkably stable and generally well-optimized. Frame rates remain smooth even when managing massive, multi-department facilities with hundreds of patients and staff. There are no major, game-breaking technical bugs; the primary "technical" friction points are tied to the inherent design of the AI and UI (e.g., pathfinding logic), which can be frustrating but do not crash the game. It runs well on older systems and has a solid history of stability since its release.
⭐ Concluding Justification
Project Hospital is a challenging game for a niche audience: those who value depth, realism, and complexity above all else. It is not a casual experience. It will punish you for poor architectural planning and demand you manually intervene in life-or-death diagnostic scenarios. The commitment required to learn its systems is significant, but the reward is one of the most comprehensive and satisfying management simulation experiences on the market. The highly detailed Pros overwhelmingly justify enduring the steep learning curve and the occasional AI frustration. If the tags "Simulation," "Management," and "Realistic" appeal to you, this is an essential purchase.

I go through phases where I still come back to this game. I like planning hospitals for some odd reason.
Incredible and very realistic game! My mother - who is a qualified nurse - played this game and said it was so realistic that she couldn't believe it!
I'm on the fence because I love the concept. It's the UI! It feels very old, unintuitive and unfriendly. I really wish they give this a refresh. Just make the UI more friendly, like Sims or Paralives or something easier to navigate. I also wanted to play as a doctor and found that confusing. I'd love to be able to create my own doctor and when I assign patients to me, they are actually assigned to me. I enjoy the Sims doctor career, but really wanted something with a touch of reality, which is why I got this. It's almost there - but the UI!!! Please come out with a Project Hospital 2 with the UI updated. Would get that in an instant.
