Playing status: 100% achievement, 3x playthroughs in every difficulty Grindy Achievement(s): No (1 achievement will require you to idle the game for a few minutes). Optional Achievement(s): Yes (12 achievements). Difficult Achievement(s): No.
Intro
Plague M.D. is a medical medicine simulator that takes place in the 16th century. Play as a healer, helping the villagers to treat their disease with unorthodox methods of the middle ages. The game gives a warning not to try the methods in real life when you start the game, so I assume you know what it means. Pros: - 5 endings - Random events that are happening every day Cons: - 3 endings will delete your save although the only difference only lies in the last two days - The game can be repetitive starting from the middle half - Sometimes, items that you placed on the table will disappear Specs Intel Core i5-9300H 2.40GHz, 8GB RAM, NVIDIA GeForce GTX 1650
Should you buy this game?
No. Although the concept is good, it suffers from repetitiveness starting from the middle half. I suggest you play other time management games instead if you like these kinds of games.

In-Depth Review

Visuals
Characters are drawn in a mix of realistic and cartoonish styles. They still retain the middle age feel, whether from the wooden buildings or the objects that are used to heal the patient. Cutscenes are drawn in the same style as the in-game visuals, with a closer look at the residents. However, the main menu looks bland with some buttons with texts that are slapped in an image. Although the image looks fitting with the rest of the game, the buttons don't blend well with the background at all.
Story
The story was told in series of still images with some text in it. The narrator's voice doesn't seem to resonate with me somehow with the overlapping voice between each dialogue. The game has 5 endings that depend on your actions throughout the game, although the story is not something that you need to think too much about.
The Game
Gameplay The game starts as a simple medicine simulator where you need to diagnose your patients and cure their disease. Most of the symptoms are obvious enough if you look at the way they walk to your clinic, although you still can talk to them just to make sure. You'll then have to concoct medicine for your patient or do an operation to "cure" them. The game is played in a day system with 11 days in total. Some NPC will appear to your clinic as you play the game. They will bother you by making you sleepy or making your patient not take their medicine, forcing you to make it all over again. This bothers me at first since I didn't know how to prevent it, but it doesn't pose a problem once I knew what to do. https://steamcommunity.com/sharedfiles/filedetails/?id=2474052535 The Cure Everything that you need to know to make a cure is listed in the reference book. You can open this book in-game, pausing the game in the process, to match the patient's symptoms with their disease and cure. The cure will be listed in a series of images of what you need to do, similar to those Artifex Mundi games where you need to concoct a potion. However, although it's quite obvious, the game never states that the scalpel can only be used on the patient - I thought that I needed to use it on the medicine instead. Since most diseases and symptoms are similar, you're encouraged to remember them in your head. The imposed time limit on each patient also helps to encourage you to memorize the solutions. The game will introduce more diseases and cures as you progress, although it doesn't take long before I got used to it. This makes the game to be repetitive, especially with the lack of challenge and the numerous patients you have to treat every day. The game seems to increase the difficulty on the last day, although it was already too late to sate my boredom. https://steamcommunity.com/sharedfiles/filedetails/?id=2474054499 Although the game starts as a game where you have to diagnose your patient's symptoms, people who want an easier time can also use tips that can be accessed in the settings. Turning the tip on will show the exact medicine that the patient need, in which you must figure out how to make them. To be honest, I didn't like this system and it only ruins my concentration when it automatically turned on in the first two days. Random Events A random event will appear after each day, allowing you to choose between 2 options that will either change your fatigue level, trust between villagers, or both. You need to keep both meters low at all times, as maxing out the bar will lead to a game over. Luckily, the game seems to be forgiving in this regard - you can spend the money that you gathered from treating the villagers to lower both meters to the minimum, and trust me, there's a lot of money to spare. https://steamcommunity.com/sharedfiles/filedetails/?id=2474053083 Length and Difficulty I finished the game for the first time in ~2h. I encountered two endings in that run, one with a bad ending where I didn't maintain my trust well enough. The game has 3 difficulties and there doesn't seem to be much difference except for a faster timer when you are treating the patient. As for the difficulty, the game might be hard to take in at first, especially since it does poorly on the tutorial - it shows how to cure someone and then present an arrow on all clickable objects, with a note for you to check them out. I didn't know that the reference book can be used to check the cure at first and only relied on the one that was taught in the tutorial. Some other mechanics like the trust meter and fatigue is also not explained very well that I didn't know how to reduce it until I looked around myself. Problems The hint button is automatically turned on in the first two days despite it showing as disabled on the setting page. Some items that you placed on the table will also disappear if you do some actions.
Conclusion
Although the game looks promising at first, it becomes boring quickly with the lack of challenge as you play the game. Diseases become easier to cure once you have gotten used to them and there's no sense of a rush to cure the patients. The last level might increase the difficulty a bit, although I found it to be annoying since the mechanic revolves around me spamming my left mouse button on an item to equip it. I wished that some days from the middle half can be cut to prevent boredom, but the game is already short enough as it is. The decision to delete your save after you reach 3 endings are also not helpful since you have to go through the same boredom three more times. The game has potential, but with it being at the current state, I'd rather not recommend it as of now.
Pros:
✓ It is extremely atmospheric and immersive. ✓ The graphics are done very well and with great care. ✓ The repetitivity of the actions can be therapeutic, but do not worry, it’s never boring. ✓ A large variety of illnesses and tools.
Cons:
🗴 Main story lasts only 2 hours. 🗴 Story voiceover is well done, but it’s in russian. Plague M.D. is an medieval doctor simulator. A brief dive in the 16th century european village in need of anybody willing to take the responsibility of becoming the doctor.
Plot
The player is put in the position of a weary traveler. After having reached a village he discovers the possibility to settle down, taking the vacant spot as the healer. Although he knows nothing on the matter, his silver tongue grants him the position. However, every slip and mistake is accounted for and charlatans are only good as stake fuel. And so as a mysterious sickness spreads around the country, this amateur doctor has to worry both about his own life and safety of the village. https://steamcommunity.com/sharedfiles/filedetails/?id=2351281951
Gameplay
There are 11 chapters in total and each consists of a day of work that in the narration is representative of a longer period of time. The chapters are divided in three parts: the time in the village in which it is possible to visit the tavern and run errands at the shop, a major event with choice and finally work. Oftentimes work will be prefaced by yet another interaction, such as lamenting of the priest or an emergency the healer is not quite ready for. The core part of the game follows a formula similar to those used in cooking games. One person in need of treatment at a time visits the facility where they explain or visually demonstrate their symptoms. The cure has then to be prepared according to the recipe book that can be consulted, which will also pause the game. Tools and medications to use are also displayed above the patient, however some steps might be tricky or look confusing, especially in the beginning. There are some elements that disrupt the routine and make the work harder. Those include rats jumping on the table and stealing ingredients that are being processed, the priest singing the healer to sleep, the drunkard who convinces sick people to not accept treatments and throwing them away and others. The mallet can help getting rid of some of those nuisances. The difficulty in the game decreases with progression. In the beginning it might feel unfair, but it doesn’t take much for the balance to shift. This happens because of the accumulation of recipe and symptom knowledge together with tool upgrades make the therapy much faster. Each chapter introduces new, more complex remedies which makes the game more dynamic and never too easy. As the plague spreads to the village, the doctor crafts the characteristic plague doctor mask. It is however very hot and difficult to breathe with it and so it has a cooldown time. Nothing happened even if not wearing it for the time required to cure a few people, but it is possible to contract the plague. Patients with plague will also require not one, but two different cures. Choices that are made during the events and dialogues have a critical role in the game because they influence the reputation with peasants. If the patience bar fills up normally the player would be sent back in time, or if the “one life” mode is active it would result in a game over. Pardons can help and lower the bar and a Royal Pardon can help evade the death toll once, but those items are costly and buying them subtracts money that could be used to buy faster tools. As this work is extremely stressful there is one more thing to take in account. Even if the healer says he’s “good as always” his exhaustion can be seen as his portrait becomes darker and darker. Stress level can be reduced at the tavern at the cost of some coins and reputation, but putting it off for too long can result in his undoing. There is also an endless mode, which is a challenge to cure as many patients as possible before collapsing.
History, Visual Style and Soundtrack
The game is drawn in a gorgeous 2d style. It resembles medieval miniatures, with heavy and detailed ink lines, with a lack of prospective depth, very appropriate for a Late Medieval Europe. However the plague doctor has first made an appearance in the century after the century in which the game is set. A very nice touch is how the various remedies have been carefully studied. For example the St. John’s Wort is a herb that has been historically used to cure depression and is still being analyzed in contemporary medicine. Wounds have been treated with egg yolk and turpentine to remove maggots. Leeches were every doctor’s favorite as well. https://steamcommunity.com/sharedfiles/filedetails/?id=2351288458 The background music is simple and fits well the setting. The tempo changes in a few instances, making it more interesting. The characters babble in a “simlish” manner, but the plot narration has been read by a voice actor. It has been voiced skillfully, however it is in russian.
Final Verdict
The game, although brief, is very enjoyable and provides a bit of escapism. Starting can be a little nerve wracking, as there will be inevitably some patients collapsing before receiving their treatment. Later on the priest and also the drunkard will be the cause of frustration. However this is part of the charm of the game, it always has a challenge for the player. I would recommend the game to those who are passionate about middle ages, medicine of the past or simply restaurant simulators. Aside from the occasional bugs this game plays out like papa john's pizzeria games. It is fun and a great concept but needs some bug fixing. I do say it's a bit high of a price for what you get so wait for a sale and pick it up.
I really enjoy the art and the concept, but the gameplay is a bit lacking. It's akin to a flash game, with just memorizing a recipe, performing it for the random sick person, rinse and repeat. For the price I would've preferred a little more complexity and variety in gameplay.
The playstyle reminds me of some sandwich-making flash games but this game hosts a much larger historical background to prevent it from being boring like those games. Mechanisms like the choice between villager's hostility and doctor's own health are also what makes this game more than just a clicking order-serving game. The story is a little bit short, though. The mechanism that the doctor has to get off his mask after a certain amount of time is really challenging when there are other obstacles such as the drunk and the beggar around, but it could be a little disappointing that such a mechanism only lasted for 1 chapter in the story mode. The story is well written and organised, but it would be nice if the story was longer. With a more complete story, this game certainly deserves much more than its current price. Currently, the game does not cost a lot and serves as a good time-killer, somehow lacking some contents but it's still a creative game.
Oyunun hikayesini beğendim. Mekanikleri birazcık basit olsada sırf oynanışına düşük derece vermek saçmalık. Çizimler oldukça iyi. İlk oyunları olarak baya başarılı buldum. Türkçe çevirisini yaparken ekiplerinin yarısının covid-19 hastalığına yakalandığını öğrendim. Buradan ekip arkadaşlarına tekrardan geçmiş olsun dileklerimi iletiyorum. 11 lira bir şey değil alın bir deneyin.
i probably would not recommend this game for a few reasons, one the description says there are lovable characters. this is untrue (at least in my opinion) as all the characters are annoying as hell. the idea is neat and i like the art style, but the game is also short, repetitive and glitchy. there is one bug that made me repeat a chapter twice. when putting the towel on the cutting board accidentally, sometimes it will not allow you to put it in the trash. which means your cutting board is occupied so you cannot use it, so your patients cannot get their medicine and will die which can lead to you being burned at the stake. it is incredibly annoying. also sometimes the ingredients would disappear even when there was no one and nothing around to steal them and the dragging was very finicky, which lead to me having to go back and forth a lot more than necessary. overall now that i have completed the game i feel very annoyed.
Plague M.D. is great! I feel like it can easily become a comfort game, the graphics are absolutely amazing and i love that it actually has a storyline and a progression and different endings, which make it different from the flash games that I've seen it being compared to. As other comments suggested, it would be cool if they had achievements like "killed 100 rats" and stuff like that, but it's more of an extra thing that would make the game even more entertaining than it is already, so I wouldn't call it a flaw. The early game achievement has already been deleted, which is great for the achievement hunters out there :) edit: Even tho I got the "Saved your own skin" final, I never got the achievement. If anyone knows how to fix it pls tell me
It's simple, it's short, but it's good. I'm loving the art style. Really brings me back to the days I used to play games like this on browser. Definitely worth its price. Edit: Just found out that, at the moment at least, there is one achievement you can't get if you didn't play the game when it was in early access. It doesn't really matter to me, but for those people who want to 100% the game - beware. Edit edit: The early access achievement was removed, hooray!
