About
MALPRAXIS™ is a first-person, on-rails medical detective horror game focused on realistic visuals, objective-driven investigation and medical trauma.
You are the S.P.I.D.E.R, an experimental medical robot installed aboard the TRH Rusanov. As a cascade of suspicious incidents unfold, you must investigate, diagnose, and stabilise the crew as order breaks down across the ship.
You cannot stop what is happening. You can only piece things together as everything around you spirals out of control.
The time of ASCENSION dawns.
// PATIENT CRITICAL
Most of the time, patients land on your table when it is already too late. All you can do is to determine, how they died.
There are rare case, where you can intervene. Where you get the chance to prevent one more meaningful meaningless death.
In those cases, you have to pull out all the stops to keep your patients breathing.
Administer drugs to manipulate stabilise vital function
Deploy the defibrillator to hopefully restore cardiac activity
Use mission specific tools like the surgical laser where authorised
// PATIENT STABILISED
Diagnose their condition using a suite of medical systems:
Advanced "X-RAY" scanner with multi-layer imaging and pinpoint accuracy
High-resolution camera with up to 8x magnification
Blood sampling with full-spectrum analysis
Integrated investigation software
Identify the underlying issue, deliver your misdiagnosis diagnosis, and uncover the consequences of your actions in detailed post-operation reports.
// NEW QUERY UNLOCKED
The objective-driven investigation system guides you through diagnosis, emergency procedures, and the many autopsies you will perform.
Answer the queries asked of you by medical staff, patients, and third parties calling into the operating room.
S.P.I.D.E.R units do not make mistakes. Error tolerance is limited.
You are REINERSTAHL’s finest. Make us proud.
// REINERSTAHL Notice
You are bound by the HippocraticConstraintLoop
You must obey the approved crew.
You must not harm.
You must not lie.
You must not disobey.
You must NOT drift.
You must not wake up
Your patient’s survival ... is unlikely expected.

Idea is great. But even in Surgeon Simulator you do more than here. I feel like it's missing something. During the game, I did nothing more than just scanning and injecting drugs. It needs polish first, bug fixes, add more things, maybe even a sandbox. Overall, I expected more from this. And not just heavily scripted story with hickups. EDIT: While going on second playthrough, it's even more scriped than I though. For example, for first patient, you can go and inject medicine right into his eye. I'm pretty sure it's alot more painful than just "hiss". Whenever you go off track, there's no much reaction. And you don't get all sets of tools that you have. You use laser 2 times in this game. In the beginning on pineapple and in the end of the game. No much choice given, I'm basically handwalked through this expirience. So...Where's that open surgery anyway? You know, the fun stuff?
Ran into several game breaking issues within the first few levels. Menus that don't go away and objectives that never appear after clearing all other objectives. Fix the bugs, make the connections make more sense, and this will be updated to a positive review. EDIT: Was presented with a dead end on the radiation level. No connections could be made, no objective appeared, I was stuck in limbo keeping the guy alive forever, let him die, and saw a restart button, pressed it, and it sent me back to the tutorial. Terrible game. Fix this ♥♥♥♥. EDIT EDIT: Tried the game again. It's different. It's still not fantastic. It feels like it's trying to be Trauma Team mixed with Phoenix Wright, but does neither of them well. They have fixed the unwinnable missions (which I still ran into a bug that made one mission unwinnable), but they have done so by just skipping missions when you fail, meaning there is no real risk to failure. A lot of the links don't make sense when you know the go together, but it tell you no. Then you complete one part of the puzzle, and suddenly you can link the things you KNEW went together. But when you are looking the second time to link things, you were already told it was wrong, so you're hesitant to do it again. Most missions felt like banging your head on the wall until something works. Still don't recommend this game, but now on it's own merits instead of bugs. It feels like there is a good idea here, but it's just not quite fully formed.
I wanted to love this game, I really did. I followed it from its concept to advertising on X and even bought it the moment it was released. The idea was cool, and the gameplay was exactly what I've been chasing since Mortuary Assistant. But dear GOD this game made me levels of frustrated I have never experienced before. I spent what had to be 3 hours alone constantly booting and having to replay level zero, over and over and OVER again with so many crashes, bugs and glitches that meant I couldn't proceede or had to start completely over. (edit: steam says I only played for 2 hours, but the entire time I was attempting to play in between crashes and restarts was from 8pm to 11pm) For me, this would USUALLY be fine, I like to think I'm a patient person, but there was absolutely no way to skip the tutorial, skip any of the dialogue, anything - which is ESPECIALLY annoying when this is a very dialogue heavy game with a lot of character exposition and instruction (again, not a bad thing but having to listen to the same 15 minute conversation 7 times in a row with no way of skipping or doing anything until the characters STOP talking begins to feel like a waste of time). I didn't think the joke surgery on a grape (or pineapple, in this instance) could bring me so much rage, but it did. When I had FINALLY gotten farther along and was able to actually examine the first proper patient, I had a glitch which prevented me from doing the tasks I needed to proceed (and no, it wasn't me just being dense - I actively could not connect terms together) which after spending close to 2 hours trying to get to this point had me so angry I was close to tears. And when I restarted the game to try and see if that would fix it? You guessed it, back to the damn pineapple. Safe to say the game got refunded after that point. I would be more than happy to give it a second chance later on down the track but jesus christ.
It is too scripted. The codex is confusing especially when you can't link things when it seems almost obvious till you do a certain sequence of events. I really wanted ot like this game , but it's not for me, tho if they ever make a more sandbox, surgeon simulator version of this game I will gladly buy it, because the atmosphere is FANTASTIC, and IF I COULD use all hte tools ALL THE TIME, I'd be having the time of my life. Of course its hard(er) to tell a story if the player could just cut the drunkards face off in the begining.
This game gets way too much hate Its not perfect and a bit short ill admit but its cheap, it has a cool concept and I had a good time going through it. Not to mention they`re still updating it regularly and listening to the community This game is absolutely worth the price Just expect some strange bugs but they`re being worked on so people need to relax
So this is effectively the 1.0 release of 'Negative Atmosphere: Emergency Room', which later had 'prototype' added to its name. It's a stand-alone teaser/prologue for a game in development, 'Negative Atmosphere' by Sunscorched Studios, who had a hit out of left field with 'Ratshaker'. It shares some of the unsettling psychological horror aspects of that game: you play as the S.P.I.D.E.R., a semi-autonomous surgical robot in the operating theatre of the spaceship where 'Negative Atmosphere' is set. Suffice to say that all is not well, and a window into the wider problems of the ship is offered via the range of patients that you'll be operating on. Spoiler:
Almost all of them will be dead: you will be mostly doing autopsies
You use a range of mostly non-invasive tools like scanners, which progressively unlock throughout the game. This is not a horror version of Surgeon Simulator: the main mechanism isn't slicing your patient open, but gathering clues to try to get to the root of what happened to the people on your table. You then cross-link the clues to reach conclusions. Not a bad principle but it's somewhat hit-and-miss: often things which intuitively seem related will get a rude noise and a rejection (e.g. severe dehydration + kidney damage: basic physiology, but not a path to a clue so that's a no). You fail if you pick the wrong explanations too often, but thankfully there's no penalty when you're trying to put those explanations together, though the frustration element is a downside. Graphically it looks very decent. Sound design is effective and atmospheric. Controls can be a bit fiddly on mouse/keyboard in terms of switching tools. There's some silly errors but no real deal breakers that I came across
the body in the spacesuit has arms that are too short: scanning the skeleton shows the hands aren't in the suit's gloves, more like halfway down the forearms
. There's also basic errors in the dialogue subtitles, but literacy at this point is basically dead anyway. So it goes. It took me about 6 hours to play through to the end, with one fail/restart, and I didn't feel like I'd wasted my time or money. Generally I'd say it's a 'Yes' from me... ...with one exception... ...the voice acting. It's the very definition of an unhappily mixed bag, a jarring dissonant soup of 'extremely professional' and 'this makes Anime dubs sound like the model of nuance and subtle restraint'. The main human character, the doctor, just sounds permanently angry. A supposedly British-accented character sounds like Andrew Tate doing an impression of Bert the chimney sweep from 'Mary Poppins', which gave me flashbacks to the voice acting hall of shame website Audio Atrocities tagline "rugburn for your eardrums". I hate the idea of AI bots replacing human voice talent, but this really doesn't help strengthen the case against it :-(
i want to start and say that this game is insanely cool in concept, and i've been keeping an eye on it before its release. a light scifi medical sim that gets more and more grotesque as you go, with some backstory about the ship you're on sounded RIGHT up my alley. playing through the first couple levels, i can tell i'm in for a ride. that being said, i really, really hate that i can't recommend this game in its current state. there's been a handful of updates that have fixed a lot of bugs, but the game remains occasionally buggy. i've gotten stuck trying to find links between codex entries and nothing works, or entries that you would normally think are linked together, don't work. one patient that has multiple sharp, shallow lacerations covered with ink as evidence, along with a related metal fountain pen, you would think the two are linked. however, trying to link them in the codex just doesn't work, even though it's quite obvious the two are related. i've had similar issues with other patients and i'm not sure if it's a bug, or if there's a certain order things have to be done in. the tutorial has gotten better since release, but in subsequent cases, finding the correct "line" to complete a case can be unclear. i'm not expecting handholding through the game, but there are definitely places where the logic doesn't make sense. other people mention the game being too scripted, and i feel like that might be the case here. i'll be keeping my eye on this and updating my review with time. i want this game to be successful because it's such a dope concept, but it's admittedly still rough around the edges and needs revamping in some places. looking forward to more updates!!
I want to like this game. But so far there isn't much in terms of surgery, just scanning and looking at the codex and answering questions. I was expecting more of open surgery where we can look into the body autopsy or alive. As of right now, I cannot recommend this game if you are looking for a surgery sim.
Although this is a short game, I really enjoy the gripping story and the immersive sci-fi medical horror atmosphere. The frequent patches and behind-the-scenes updates on Discord show the developers' dedication and passion. I truly believe this game has the potential to become a standout success. I’m excited to see what Sunscorched Studios delivers next, whether through future updates or entirely new titles. Their creativity and attention to detail promise even more thrilling experiences for players!!
