One of 4 controllers is always broken, no matter what input settings are used. It's not a specific controller issue either as reordering the controllers changes which one is broken. Keyboard for 4th player also cannot be used, since one of the controllers seems to overlap with the keyboard and take control away from it.

Phenomenal co-op game. Ignore negative reviews. Great level design, character customizations, and themes. 5+ hrs to complete the full game as well as a fully fleshed out versus mode.
