In this visual novel-style training simulation, you will build relationships with eight TGD patients across eighteen clinical encounters, for a total of 3+ hours of gameplay.

As a new provider, you will prescribe and optimize hormone therapy, counsel patients around surgical affirmation, navigate care considerations for TGD adolescents, and more! The decisions you make will affect the patient’s trust and stress levels in real time.

Receive training and mentorship from the clinic’s attending physician, Dr. G., and get debriefed on your performance after each encounter by the clinic’s director, Dr. Jay. You will be able to replay each encounter to your—and your patient’s—satisfaction!

Each of our 18 simulated case scenarios are informed by the lived experiences of our consulting LGBTQ+ patients, health providers, and reviewers and aligned with evidence-based practices. To bring these characters to life, our teams made an open casting call for TGD visual artists and voice actors, and hired identity-aligned creatives to portray these characters.
By the end of this training, it is our goal that players will be able to:
Demonstrate inclusive, patient-centered communication strategies that affirm the identities and experiences of transgender and gender-diverse individuals across the lifespan.
Apply knowledge of gender-affirming medical therapies — including testosterone and estradiol regimens — to support individualized transition goals while managing associated clinical considerations.
Identify ethical, legal, and institutional considerations in the care of transgender and gender-diverse patients, including confidentiality, consent, and documentation practices.
Describe the implications of gender-affirming care on reproductive health and fertility, and counsel patients on preservation options and long-term planning

This game is especially good If you want to know what to expect from a doctor visit discussing gender affirming care or are curious about hormone therapy. You play as a student doctor and gameplay consists of choosing dialogue during patient visits. Before each visit you can get advice from Dr. G who will give some examples of dialogue options. At the end of a visit you can see if you made all the key choices. The most difficult choices involve figuring out how to communicate your evaluation of how different treatments would work for the patients' health and habits based on their different personalities Seeing all their life stories and how they progress was fun. It's really cool that something like this is free.
I wanted to play this awesome game with my jewish transfemme girlfriend and her Angolian bull, but this game doesn't run on my antifa-approved Linux distribution. This goes against principles of inclusivity, that this game pushes. I hope this bigoted policy against Linux users of color will be ended. Downvoted.

This game is propaganda with a medical coat. You’re a new doctor at a clinic and the whole point is to push hormones and surgeries on patients as fast as possible, including teenagers. Say the "correct" affirming lines, raise their trust meter, and you’re a good little provider. Question anything and you get a bad debrief. No real discussion of risks, sterility, long-term outcomes or detransition. Just smooth the path and get them on the conveyor belt. Feels less like medical training and more like a simulator for turning kids trans as efficiently as possible. Almost like early intervention is the whole point. SHUT IT DOWN!!!

I appreciate this opportunity to see a more ideal care system as a trans person. In the country I am currently living in just starting access to healthcare as a trans person is more like a 3+ year nightmare unless you are wealthy enough for private healthcare here AND from other countries. The only bug I experienced was saving the first time when you are asked if you want to save (just select no and then save manually, every other time should be fine). Also I would like to say to other trans people to be careful to take breaks with this game when you need them; I haven't finished this game just yet, I think I am about halfway done. Lastly, every form of systemic inequality in healthcare is horrific and unacceptable! Millions of people and their suffering and preventable deaths being dismissed, neglected and abandoned is unbelievably cruel. That is why the feeling of trust and safety in healthcare is life-saving and why very many structural changes are still needed in most countries, which is one of the things this game is showing. A better future starts with people changing the present, not by waiting for others to do everything, denying other's reality or siding with cruelty.
No game that promotes the mutilation of bodies of people struggling with mental issues deserves a platform to stand on. This game is about grooming users, especially children, into sterilizing themselves. The game abuses people with body integrity dysphoria into cutting off a part of their organs and tissue. It's not cute. It’s predatory. It takes players already drowning in mental illness, body dysmorphia, trauma, identity collapse, untreated psychosis, and grooms them to do permanent damage to their own bodies. It dresses irreversible harm in the language of “agency,” “expression,” or “choice,” then laughs when the player bleeds for it. That is not liberation. That is productizing suffering. Mental illness is not a costume. Compulsions to destroy healthy tissue are not a quirky side quest. A title that frames surgical or self-inflicted ruin as the heroic path for characters defined by psychological distress is not brave storytelling. It is marketing cruelty to an audience that already struggles to distinguish fantasy from the urge to act. SHUT IT DOWN!!!

The game infantilizes lgbt people because obviously they need to be quirky and silly. The game has some important information (side effects, planning) but also promotes unhealthy ideas such as having testosterone prescribed on the first visit without prior mental evaluation. It also promotes treating people like they are made of glass and get offended extremely easily, it looks like it tries to justify such behaviour as normal. Generally expected worse but it needs work. As of right now it further represents lgbt people in a bad light.
