Endorphin Vice runs one of the most advanced ragdoll systems in any sandbox game. Bodies react. Every character is built with precise, anatomically distributed weight, so physics behave exactly as they should under impact, gravity, and force.
Enemies collapse, stumble, grab at walls, and hit the ground differently every single time depending on the caliber, velocity, and angle of the hit. Critical impacts to the head or neck can trigger brain aneurysms, sending targets into a ragdoll coma. Cardiac arrest from extreme stress forces a full ragdoll collapse. Even the player ragdolls and can recover.
Every shot generates procedural mesh flesh deformation in real time. The wound reacts to exactly where you hit, what caliber you used, and how fast the round was traveling. The body bleeds out dynamically, staining walls, floors, and the surrounding environment with spreading blood pools that grow over time. New blood splatter tech interacts with surfaces and geometry realistically.
Dismemberment logic reflects the actual physics of the impact. High-caliber rounds penetrate and can take multiple enemies in a single shot. Gore System 2.0 fixed all legacy clipping issues and pushed the brutality further than ever before, including a dedicated chainsaw with its own execution animations and a fully overhauled machete suite.
Every punch carries real physical weight. Hit enemies until they bleed from the nose. Drop them with a knockout. Then walk up to a downed, ragdolled enemy and finish it with a head stomp.
Throw knives and axes with ultra-realistic physics. Weight, rotation, velocity, and impact are all fully simulated. Get close enough and the character automatically stabs downward, pinning the weapon into the target. No scripted hits. Physics only.
Agony System: visual shock post-impact, permanent blur below 25% HP
Cardiac Arrest: exceed 240 BPM from stress, bleeding, or drug abuse and your heart fibrillates. Instant clinical death.
Brain Aneurysm / Coma: critical head impacts trigger neurological overload and ragdoll coma states
Tourniquet: stops arterial bleeding instantly, but spikes your BPM
Adrenaline Syringe: ignore pain and sprint with a shattered pelvis. Spam it and die of a drug-induced heart attack.
Mannitol (Cranial Auto-Injector): the only way to pull yourself out of a coma
TACTICAL AI
Civilians react. If you start throwing hands in the streets, some will swing right back. The AI has been entirely rebuilt: enemies hear gunshots, footsteps, and impacts. They investigate last known positions, search dynamically, flank, and hunt. Police units follow strict behavioral logic. Civilians flee to the safest available position. NPCs who get shot will try to grab your barrel before they go down.
Every bullet is a physical projectile. Real gravity drop, wind deviation, material-based penetration, and true travel time. A real-time ballistic HUD displays live calculation data. High-caliber rounds punch through cover and chain into multiple targets.

Saw the game on tiktok and it looked fun but the gameplay the studio shows is NOT what the game provides and its misleading at best. The game is clunky and almost unplayable and the menu doesnt work and the game locks itself or spazzes out when you simply press ESC. there is no volume control or simple grapichs settings even the gun play is weird and mechanics are working against the player rather than with. any kind of reloading makes the character to point the gun down and it wont automatically point it up you need to press the mouse button to aim again to make the gun point upwards. In conclusion the studio is more or less not truthful with the state of this game and i believe the game should not be on steam it should be in the studio being worked on because the state of the game is not even BETA worthy. DO NOT BUY
This game is, unfortunately, an ultimate example of feature creep. I bought this game quite a bit ago, and in the time since, there have been some good additions- New weapons, a new mission or two, etc. Nice stuff that enhances the actual gameplay experience. On the other hand, it's also been plagued with additions that don't really enhance the experience and seem to take away from actual development resources. A menu update that, for some reason, changed everything to have a strange late 90s aesthetic. An equipment slot system that seems to bug some weapons to being unable to be reloaded. Reworks that took content that worked (such as missions) and made them buggy messes with 'enhanced' AI that now just auto-lock onto you through walls and bum rush your location, usually ending up killing most of their teammates from their allies wandering into crossfire, or because the only logic the enemies have are 'snap to' and 'shoot', without at all checking if there is an object or even another NPC in their line of sight. Another update that causes a bug where enemies, once killed, clip into the ground and make a skyrim style physics collision noise. Enemies snap to you from the first floor and then mindlessly empty their guns into the ceiling. When right clicking to aim or left clicking to shoot, your mouse is NOT Locked to the screen, no matter what your settings. You WILL die in a firefight because the game decided you were trying to click out of the window. And instead of fixing these issues, a weird super hero mode was added (presumedly because the dev watched the series finale of The Boys or wanted TikTok attention). This mode is completely broken and crashes within a few minutes of enabling. An update to fix this was promised a month ago, and then never manifested. Worst of all, Staff know these issues exist. They know old content is being broken by new updates, but when asked on Discord when they'll be reworked, the answer as 'they probably won't be. Nothing needs fixing. It's your PC.' So despite absolutely loving this game, I really can't recommend it.
I would like more maps, resturants, offices, and more guns, But thats just me being ungrateful. I like this game its realistic and I like each update making the game better

I don't know i like the physics in the game just maybe not the active ragdoll when they get shot also its hard to see the gore for me it just looks like only blood no gore then also spawning npcs and gun take too much time i will give this a negitive review and i would not recommend but you can get it to try it out but i suggest getting this game when its completed right now its not all that good tbh just get people playground or overgrowth both good games
i regret spending my sweet money on this,menu is super weird,ragdolls get stuck in the road on every map,ive expected better gore but its alr,it has okay physics,i also dont like the map system,game was cool like a couple of days when i bought it,but after trying every map and most of the guns it becomes rlly boring,ofc i know that there will probably be some people that like this game but they will also say that its mid,its nothing like they show it on tik tok
Game runs smoothly without the huge bugs and lags as described in other reviews (Maybe haven't experienced them yet) only few minor bugs that I noticed. Sometimes npc's are unresponsive, reloading have some bugs and few maps have performance issues but other than that enjoyable and playable.
this game is fun for the first 29 minutes of it its not that bad it jsut get kinda repetitive it is kinda hard to play on a rog ally but i would say its a decent game only get it of you like sandbox games <3
