This game is alright, but the PC port is so bad that FromSoftware was right to delist this from Steam. The main reason to play this game is to experience an early version of the QTE system that would later be used in Sekiro and inspire Lies of P. Ninja Blade divides its regular combat from its QTEs, where FromSoftware later integrated them together in Sekiro, which utilizes a default deflect QTE, with additional jump, counter, and dodge QTEs when a symbol displays on screen.
Poor PC Port Issues
Here are a few issues I ran into trying to play this game.

Change display setting - game crashes
Change max FPS - game doesn't save it
Change max FPS - games saves it, but reverts back to 30 max next time I start the game
Change keybindings - reverts back after restarting game
Skip cutscene - game crashes
Retry QTE - voice audio drops out
60 FPS max - "mash button" QTE in Mission 1 impossible with keyboard
It took me nearly 3 hours to get through Mission 1 with all of the crashes, restarts, and time I spent in the settings.
Game Review
If this was FromSoftware's attempt to do a Ninja Gaiden-like, it's a really poor one. The game moves about about 1/4 the speed of the first two modern Ninja Gaiden games, and the 30 FPS makes it feel even slower. As is, this game is skippable except as a relic of pre-Souls FromSoftware games. Also, if you ever wondered what game could bridge the gap between the combat pace of King's Field and Ninja Gaiden, here you go.
Difficulty
This game is not as challenging as later games in the Soulsbladebornekiroringcorerdtree series (Dark Souls, Sekiro, Elden Ring, etc.), but this allows its QTE-based gameplay to be more dynamic. Later on, Sekiro established the Masochistic Memory-Based Quick Time Event (MMBQTE) genre, focusing more on brute force memorization.
Conclusion
Hopefully, FromSoftware eventually remasters some of their older games for the PC. This one badly needs it, but the King's Field series should be remastered first. Overall, it's not a horrible game, but not a great one, and the non-gameplay issues drag it down further. If anything, maybe owning this version will reward me with a $4.99 discount on the $69.99 remaster in 2037, which will have exactly the same graphics, worse lighting, but maintain a solid 32-58 FPS with crashes only every 2 hours or so.

A hidden gem, I could say for sure. It's a strong slasher-type game with admirable QTE sequences that make every episode epic. Something needs to be done with it, like remastering or a complete remake, but I guess this game has been completely forgotten. Yes, it runs very poorly, but you just need to bear with it.
It's a little buggy, but it's still unique enough that you should check it out if you like games like Wanted Dead, Binary Domain, or Bullet Witch. If you like games with that 7th console generation vibe, Ninja Blade is for you.
Game-breaking bug on the first level. I cannot escape the building. Holding RT the entire time, no matter how fast I go or how clean my jumps are, the floor gives out near the end and no amount of jumping, Ninja Vision, or running gets me past. My character falls through the first later of rubble, clings onto an invisible object while the floor falls out from underneath of me. My character then hoists themselves up on this invisible object, only to plummet to their death. Game is delisted for a reason.
GAIDEN SOULS LIKE QTE MASTER Playing this game makes me realize that Dark Souls is a game for casual gamers. Works perfectly
